Per Ohio Revised Code, every minor 14 through 17 years of age must have a working permit unless otherwise stated in Chapter 4109. For additional information on restrictions, prohibited acts, and wage agreements, you may read the ORC Chapter.

To obtain a minor work permit, students/minors should use an application form to gather the necessary information and approvals from their potential employer and from their parents or legal guardian. They must also have a physical performed by their physician and the Physician's Certificate completed. Physicals are valid for one (1) year. If the student has a current physical, a copy from the physician's office may be submitted in lieu of the Physician's Certificate. 

Once these forms are filled out completely and signed, they should be brought to Abbie Cain in the District Office where a permit can be issued based on the information on the form. Anyone (student, employer, or school personnel), may print a copy of the application and physician form to begin the process of obtaining a work permit.
